Intake is the first real conversation: the point where a person experiencing homelessness in Vallejo is listened to, recorded accurately, and connected to the housing, ID recovery and support services they actually need.
Pathfinders for Hope is a Vallejo, California nonprofit building that access point for Vallejo and Solano County. Nothing is operating yet. This page describes the intake the Vallejo pilot is being built to deliver, and what it will look like when it opens.

Intake is not a form to be processed. It is a conversation in plain words about what you need and what is standing in the way. What you say is written down once, accurately, so it does not have to be repeated at every door you are sent to.
Three commitments are being built into it from the start. No record is created without your consent, and you can decline any part of it and still get help. No government ID is required to be seen. And no case can be closed into an unknown destination.
That last commitment is the reason this work exists. California's own State Auditor found the state stopped tracking outcomes in 2021, and that nearly one in three people left programs for a destination nobody recorded. Identification. Not having a government ID is not a reason to be turned away. ID recovery is treated as a first step, not a prerequisite, and the intake records what you are missing so the work of replacing it can start immediately.
Housing. Where you are sleeping now, what you need next, and which housing route actually fits your situation rather than whichever one happens to have an open slot.
Resource matching. A connection to the organisation that can help with your specific need, whether that is food, medical care, benefits, documents or something else entirely.
A confirmed handoff. A referral is not finished when it is sent. It is finished when the receiving organisation confirms you arrived. Sent is not the same as served.
Adults, families, and anyone experiencing homelessness or at immediate risk of it in Vallejo and the wider Solano County area. There is no sobriety requirement, no address requirement, and no requirement to have a phone of your own.
