A person is being discharged from a Vallejo emergency department with nowhere to sleep that night. Here is what a warm handoff looks like, step by step.

The hospital social worker asks permission to share information with the receiving shelter, and explains exactly what will be shared and why.
- 2In the room
The social worker calls the shelter's intake staff with the person present, introduces them by name, and lets them hear the plan.
- 3Before they leave
The person walks out with the program name, the address, the time to arrive, and what to bring. Written down, not remembered.
- 4Same day
The receiving team confirms the person arrived. The status changes from sent to received.
- 5If it stalls
No confirmation comes back, so the sending team does not assume. It calls, records the barrier, and tries the next option.

The term comes from clinical care. The Agency for Healthcare Research and Quality (AHRQ) describes a warm handoff as a handoff conducted in person, between two members of the health care team, in front of the patient and family. It is common when a primary care clinician introduces a patient to a behavioral health provider during the same visit.
