What referral accountability means in housing and homelessness work
Referral accountability means that partners can see whether an authorized referral arrived, who owns the next action, what response occurred, and whether a capacity or eligibility barrier stopped progress.
It does not promise housing, placement, eligibility, or acceptance. It gives participating organizations a shared way to see where a handoff moved forward and where the pathway broke down.

A four-step accountable housing handoff
Each step names what partners should be able to confirm. This is a planning model, not a claim that the Vallejo pilot is live.
01 · IDENTIFY
Need and permission are clear
The sending partner confirms the person’s stated need and the permission required for an organizational referral.
02 · SEND
A receiving partner is named
The referral goes to a specific organization with the minimum information needed to respond.
03 · RESPOND
Receipt and next action are visible
The receiving partner records acknowledgment, outreach, a decision, or a capacity block.
04 · FOLLOW
Outcome or barrier returns
The sending partner can see an appropriate status and help plan the next action when needed.
