Youth Services Referral Tracking in Vallejo
A consent-aware framework for schools, youth programs, family supports, behavioral health teams, and community organizations preparing to confirm cross-organization handoffs.
PRELAUNCH: Operation Iron Gate is being developed as a North Bay Pilot. Public intake is not open, and this page does not promise placement or services. GATEHOUSE, our community platform and a smaller version of Operation Iron Gate, is open now: register on GATEHOUSE.
PLAIN-LANGUAGE DEFINITION
What responsible youth referral tracking means
Youth referral tracking means that authorized partners can see whether a referral was received, who is responsible for the next action, and whether contact, a service decision, or a barrier was recorded.
The workflow should use the minimum necessary information, respect the permissions that apply, and keep safety and appropriate adult involvement central to every handoff.
PLAIN-LANGUAGE PROCESS MAP
A four-step youth services handoff
Each step names what partners should be able to confirm. This is a planning model, not a claim that the North Bay Pilot is live.
01 · CHECK
Permission and safety come first
The sending partner confirms the appropriate consent, permission, and safety considerations before sharing information.
02 · SEND
The referral has a clear owner
The receiving program and requested next action are named so responsibility is not ambiguous.
03 · ACKNOWLEDGE
Receipt is recorded
The receiving program confirms arrival and records who will take the next action.
04 · UPDATE
A safe status returns
An appropriate status, barrier, or next step is shared back without exposing unnecessary personal information.
OUTCOME MEASURES
What youth-serving partners can measure
These are definitions for future reporting. No verified pilot results are available yet.
MEASURE
Acknowledgment rate
The share of authorized youth referrals that the receiving program confirms it received.
MEASURE
Time to first contact
Elapsed time between receipt and the first documented, appropriate contact attempt.
MEASURE
Barrier categories
Aggregate counts of eligibility, permission, safety, no-response, and capacity barriers.
Questions for a youth referral workflow review
• Which permission and safety rules apply before information is shared?
• Who can see each status, and what details are not necessary for the handoff?
• Who owns follow-up when a youth or family cannot be reached?
• How will partners document capacity blocks without making sensitive information public?
The working room
Don’t fix referrals alone.
GATEHOUSE is where the agencies, hospitals, churches and neighbors working on referrals in Vallejo and Solano County talk it through. It is the part of Operation Iron Gate that is open today, and what members say decides what we build next.
