What is Operation Iron Gate?

Operation Iron Gate is a referral-handoff and accountability model being developed by Pathfinders for Hope for a Vallejo pilot. It is designed for participating organizations that send or receive authorized referrals. Public intake is not open.

How does Operation Iron Gate work alongside HMIS?

HMIS means Homeless Management Information System. An HMIS supports homelessness-response data and reporting. Operation Iron Gate is being developed to support confirmation and status visibility across an authorized organizational handoff while each partner keeps its existing responsibilities, systems, eligibility decisions, and service decisions.

How do partners confirm a handoff?

A handoff is confirmed when the receiving organization records that the authorized referral arrived, identifies responsibility for the next action, and returns an appropriate status such as outreach attempted, decision recorded, barrier documented, or next step identified.

How do you protect privacy?

The developing model is based on appropriate permission, minimum necessary information, role-based access, responsible partner practice, and aggregate public reporting. Public results will not identify individuals or publish referral-level personal information.

What happens when a provider has no capacity?

The receiving organization can document a capacity block and, when appropriate, identify a next action. Aggregate capacity patterns can then be reviewed without presenting the blocked referral as a successful outcome.

Does referral tracking guarantee housing, placement, or services?

No. Participating organizations keep their own eligibility, capacity, clinical, housing, and service decisions. Tracking makes the organizational handoff and its status more visible; it does not guarantee acceptance, placement, or service availability.

When will Vallejo pilot results be available?

Verified results are not available yet because the pilot is still being developed. The public dashboard is online now with metric definitions and will be updated only after a reporting period, denominator, and data-quality review can be stated clearly.
