City file 07 / 36 · For Fremont organizations

Fremont referrals should match the need

Fremont homeless services referrals are logged as sent when a caseworker sends someone to another organization. Whether the person arrived is often never recorded. Operation Iron Gate, from Pathfinders for Hope, is built to record that part. Fremont organizations can watch its first pilot in the open.

Sent is not served

One Fremont referral, two records

The plain terms

What Fremont organizations should know

What it is
Referral tracking software. For a Fremont handoff it records whether the person arrived, not only that a referral was sent.
Who it is for in Fremont
Agencies, nonprofits and public offices in the City of Fremont that send referrals, receive them or answer for the outcome.
What it does not replace
Intake, assessment or anything Alameda County Health's Housing and Homelessness Services already runs. Fremont services, clients, funding and data stay with the people who hold them now.
Where it stands
Not launched anywhere. The North Bay Pilot in Vallejo comes first, projected for January 2027. No Fremont date has been set.
Who is building it
Pathfinders for Hope, a veteran-owned Vallejo nonprofit, with its board, partners and volunteers. More on Operation Iron Gate.

The rulebook

Four rules the pilot is built on

  1. No ID at the door

In practiceA missing ID does not end a Fremont handoff. The referral stays open while the ID problem is worked, with no promise of same-day eligibility, housing or a bed.

  2. Yes by default

In practiceA “no” from a Fremont provider needs a written reason before it is recorded as a denial.

  3. No dead ends

In practice“Unknown” cannot be picked as the final outcome of a Fremont case. Someone owns the next step until it is known.

  4. Tracked handoff

In practice“Sent” and “arrived” are two statuses. The sender records one; the Fremont organization that receives the person records the other.

The failure points

Where Fremont referrals break

Exhibit Fremont · Living in vehicles

Most people outside live in a car or RV

In Fremont's 2024 count, 612 people were unsheltered and 195 sheltered, and 62 percent of the unsheltered were living in a vehicle or RV. A referral for someone who sleeps in a car has to reach a person who moves.

The way in

How Fremont organizations can get involved

Three ways in for Fremont organizations. None of them asks you to change the system you use today.

  1. Watch

    Follow the North Bay Pilot

Housing held at 90 days is the lead measure. Results go on The Glass with no names, good or bad, so Fremont can judge them.

  2. Talk

    Request a briefing

Tell us your Fremont role: sending, receiving, providing or overseeing. Nothing is signed in a first conversation.

    Your open-loop count

The questions

Operation Iron Gate in Fremont: questions

Is Operation Iron Gate running in Fremont?

No. It has not launched anywhere. The first pilot is being prepared in Vallejo, and no Fremont launch has been announced.

What did the latest count find in Fremont?

The January 2026 count for the City of Fremont recorded 852 people homeless in Fremont. Up from 807 in 2024, while Alameda County fell 13 percent. Fremont holds about 10 percent of the county's count. Fremont counted 1,026 people in 2022. The 2026 split between sheltered and unsheltered has not been published yet.

What else is happening in Fremont?

South County, which includes Fremont, counted 1,064 people, against 1,074 in 2024. Fremont's Housing Navigation Center, open since September 2020, had housed 293 people by June 30, 2025, and 60 percent of the 258 people who left it moved into stable housing. The city also runs a Safe Parking Program for people living in vehicles and a Mobile Evaluation Team with police, Human Services and Washington Hospital.

Can a Fremont organization take part?

Cohort 01 is recruiting in Vallejo. A Fremont organization can request a briefing now. Nothing is signed in a first Fremont briefing.

Does this replace Alameda County's system?

No. Operation Iron Gate is not a replacement for Alameda County Health's Housing and Homelessness Services, the Fremont Family Resource Center or any provider's own records. It is built to confirm that a referral between organizations reached the person.

I need help myself. Where do I go?

For food, the Alameda County Community Food Bank lists distribution sites across the area. Operation Iron Gate is not open for public intake yet. GATEHOUSE, our community platform, where Pathfinders for Hope does your intake, barrier review and the calls to agencies, is open now: register on GATEHOUSE to ask questions and follow the build. Joining GATEHOUSE does not put you in the North Bay Pilot, which runs only in Vallejo and Solano County.
