For Vacaville organizations
A food site can tell someone where to go next. Nobody checks whether they got there. Operation Iron Gate is the referral-accountability pilot Pathfinders for Hope is building in Vallejo, in the same county, so a referral counts as finished when the person arrives, not when it is sent.
Before anyone can be tracked to a resource, someone has to know the resource exists. Pathfinders for Hope is building a community resource map for pilot planning in Solano County. Vacaville pantries, meal programs and every other door people are sent to belong on it. Get on the map.
The measurement standard is published before the pilot begins. Housing retention at 90 days leads, with checkpoints at 30, 60, 90 and 180 days, and the results go public whether they are good or bad. See the pilot.
It does not replace your services, take your clients or route your funding. Your data stays yours and can be exported. Want to talk first? Request a briefing, or read what Operation Iron Gate is.
